Medical Insurance

PMC: 100% with no deductible, including PMC Physician Offices

Employee Premiums:

  • Single - Free!
  • Family $ 88.88 per month
     

Prescription Card

  • PMC Pharmacy: $ 7.00 copay for generic; $ 20.00 copay for brand name
  • Restat Network: $ 10.00 copay for generic; $ 50.00 copay for brand
  • 30, 60, or 90 day supply at PMC outpatient pharmacy for maintenance medications for applicable copay
  • Diabetic testing supplies and select other medical supplies - Free!

Physician Services

  • Physician Office Visit Only (excluding diagnostic services)
  • PMC Employed Physician Office Visits: - Free to Employees!
  • CHA Network: $ 10.00 copay with no deductible
  • Out-of-Network: 50% after deductible

Dental Insurance

  • Preventive Procedures: 100%
  • Basic Procedures: 80/20% after deductible
  • Major Procedures: 50% after deductible
  • Orthodontics: 50% after deductible (for dependent children only)
  • $50 single/$150 family calendar year deductible

Maximum Benefit Payment (per individual)

  • Preventive, Basic, and Major (in combination): $1,000 per calendar year
  • Orthodontics: $2,000 per lifetime
  • Employee Premiums: Single $ 0.00 per month
  • Family $15.80 per month

403(b) Retirement Plan

  • Matching contribution of $0.50 on the dollar up to 10% of pay
  • Eligible for matching after one year of service

Vesting schedule:

  • 20% after 2 years of service
  • 40% after 3 years of service
  • 60% after 4 years of service
  • 80% after 5 years of service
  • 100% after 6 years of service

Long Term Disability

  • Monthly benefit equal to 60% of covered monthly earnings
  • Maximum monthly benefit of $11,000
  • Elimination period of 90 consecutive days of total disability

Life Insurance

  • Basic Life: One times annual earnings
  • AD&D: One times annual earnings
  • Supplemental Life: One times annual (voluntary coverage)

12 Paid Sick Days Annually

Paid Vacation (Call to inquire about accrual rates)

Holiday Bonus

(hourly positions only)

  • Time and one-half and plus a paid day off; or
  • Double time with no additional day off
     

(2) Personal Days Annually
 

Death Benefit

(effective May 2007)

  • 90 days salary payable to estate
  • 90 days medical/dental insurance continuation for covered dependents
